Healthy and Active Rehabilitation Programme (HARP)

HARP is for people in Ayrshire who are affected by more than one long-term condition.

We provide education, advice and support so that you can achieve and maintain the best possible health.

You can be referred to HARP by a health professional if you stay in Ayrshire, as mentioned above, the team is happy to accept people with multiple long-term conditions, of which one of them must be;

  • Newly diagnosed angina
  • Atrial Fibrillation
  • Permanent Pacemaker
  • Cancer (within a year of finishing treatment)
  • COPD (chronic lung conditions)
  • CVA (stroke)
  • Diabetes
  • Falls

At assessment you and a member of the team will discuss your health and lifestyle. Together, we can come up with a plan to help you self-manage your health conditions. This is achieved through a variety of options;

  • advice and education
  • home exercise programmes
  • rehabilitation classes
  • sign-posting to useful resources

The aim is to focus on changes to lifestyle and fitness to improve quality of life.

We work closely alongside other professionals and organisations across Ayrshire and can refer you to the service that best suits your needs and abilities.

HARP Covid Rehabilitation

Covid rehabilitation for people affected by Long Covid has been funded in Ayrshire and Arran for three years (2023-2026).  The programme is for people diagnosed with Long Covid and requiring support to self-manage their symptoms.

The team assesses and offers specialist input depending on each individual’s needs, for symptoms such as;

  • Fatigue
  • Breathlessness
  • Pain
  • Brain fog
  • Reduced Activity

To access this service speak with your GP to ensure your symptoms have been investigated prior to referral.

HARP Post Viral / ME/CFS Service

The Long Covid Team has received temporary additional funding to expand the service and include individuals formally diagnosed with conditions such as ME/Chronic Fatigue Syndrome. This change will take effect from 1 April 2026.

Referrals to the service will continue to be made via GP only.

The HARP Post Viral/ME/CFS Service provides specialist assessment, management and support for individuals experiencing prolonged fatigue and related symptoms following viral illness—including post‑Covid symptoms and ME/CFS.

Using a biopsychosocial, patient‑centred approach, the service aims to improve quality of life, daily functioning, and self‑management. The team delivers comprehensive assessment, supports treatment planning, and provides ongoing guidance to help individuals manage symptoms, promote recovery, and enhance participation in everyday activities. Care is informed by evidence-based interventions and multidisciplinary working.

Our Multidisciplinary Team

The service is delivered by:

  • Advanced Practice Clinical Nurse
  • Advanced Practice Occupational Therapist
  • Advanced Practice Physiotherapist
  • Clinical Psychologist

Clinics are delivered pan‑Ayrshire and can be offered face to face, virtually, or by telephone.

Service Delivery

Most consultations are offered on a one‑to‑one basis. We also provide a ‘Breathe Well / Move Easy’ group class, which patients consistently report as highly beneficial.
