About the Service

This service is run by Primary Care Staff Nurses who provide assessment, treatment, preventative care, screening, long-term condition monitoring and patient education mainly in a Primary Care setting.

The Primary Care Staff Nurses participate in public health and well-being interventions by identifying and promoting the health and social care needs of the local population in partnership with colleagues, patients, careers and the local community. This service is supported by a robust clinical governance framework across all services in line with local and national strategies and priorities.

What the Service Looks Like

This is a 1-2-1 service focused on long-term condition monitoring. The Primary Care Staff Nurses will carry out clinical observations and take some blood samples (phlebotomy). They will also be able to provide some brief intervention education and can help with signposting to other services.

How Do I Access this Service?

This is an appointment-only service. Appointments are organised by Dalmellington Health Centre.
