NHS Ayrshire & Arran have a dedicated team of Community Diabetes Specialist Nurses (CDSNs).
Community Diabetes Specialist Nurses (CDSNs) play a vital role in delivering high-quality, person-centred diabetes care within patients’ own homes. Care is provided to individuals who are unable to attend the hospital setting, allowing accessibility and continuity of care.
This includes:
- older adults who are frail
- people with disabilities
- those receiving palliative care
- residents in nursing homes
CDSNs provide specialist clinical input, education and support to:
- patients
- carers
- other healthcare professionals
They also extend outpatient services into the community by offering clinics closer to patients’ homes.
