Connect Me: information for practices

Connect Me supplied by Inhealthcare is a national digital platform that can be used for remote long term condition review and blood pressure monitoring.

It has a multichannel approach which means patients can choose the communication method that suits them best – for example:

  • through the app
  • SMS
  • online
  • automated phone call

A simple way to catch up with annual reviews of your patients

The platform can integrate with existing clinical systems such as Docman. There is the ability to bulk upload lists of patients or add individuals at opportune times such as during a medication review.

When a patient completes a digital questionnaire, a report is generated within the system and your practice will receive a copy of the report into Docman as an electronic document (EDT).

The completed questionnaire will be sent to your practice by the next working day.

Patient information at your fingertips

You can triage patients based on the data you receive, so they see the right person in the right place at the right time.

This reduces any unnecessary appointments and makes the best use of everyone’s time.

All information is saved in the patients’ record, so a complete history is available to all health professionals within the GP practice. The information is returned electronically with any health information needing early attention in the alert box at the top. This information allows for standardised coding and supports data entry by the most relevant member of staff.

There are algorithms to calculate common clinical scores, such as CAT scores, and identify relevant risks such as SUDEP ones for people with epilepsy.

Connect Me is currently used for the following:

Giving your patients access to accurate health education

If your patient completes any of the above pathways, they are also given access to:

  • Long term condition specific health education
  • Signposting to local services and support
  • Lifestyle advice
  • Supported self-management plans
