Connect Me, supplied by Inhealthcare, is a national digital platform that can be used to share health information with your GP practice remotely – that is, without you having to make an appointment to see your GP.
Depending on your condition, this could be through:
- the app
- SMS text message
- online
- automated phone call
How does it work?
Your GP or health professional will decide how often you should send through your information. Your chosen device will remind you. You also have access to lots of helpful information about your condition.
The information you send through is monitored, and only when necessary will your GP practice ask you to attend in person. This reduces any unnecessary appointments and makes the best use of everyone’s time.
All information is saved in your record so your complete history is available to all health professionals within your GP practice.
Connect Me is currently used for the following:
- Blood pressure diagnosis and monitoring
- Asthma review questionnaire
- Contraceptive pill review questionnaire
- Chronic Obstructive Pulmonary Disease (COPD) review questionnaire
- Depression review questionnaire
- Epilepsy review questionnaire
- Multiple long-term conditions review questionnaire
